Terms
The following Terms and Conditions govern all GP DEVA VIP membership
cards, including DEVA Star Card, DEVA Joy Silver Card, and DEVA
Vitality Gold Card.** Use of the membership card at any time
is subject to the terms and conditions herewith.
- In order to apply for a GP DEVA VIP membership card, you
must purchase at least one GP DEVA Signature product. Please
refer to the Bonus Points Table for a list of GP DEVA Signature
products. The list is subject to change with or without
notice.
- Please complete the Member Profile Form accurately to
avoid any inaccuracies which might lead to invalidation
of your membership. Enroll membership in your own name-one
membership account per customer. You may not share your
account with or transfer it to others.
- GP DEVA VIP membership card is not a credit card; it
has no financial functions.
- As a GP DEVA VIP member, you are entitled to earn bonus
points on every purchase you make. The earned points are
used to redeem your GP DEVA VIP membership. Any unredeemed
points will remain valid for a full year from the date your
new membership takes effect.
- Please present your membership card when making all purchases
so as to ensure you receive all the card's benefits.
- Please refer to the Bonus Points Table for the number
of points earned for each purchase. This table is subject
to change with or without notice.
- Membership benefits (discounts and bonus points) may
not be available on special promotion offers. GP DEVA reserves
the right to change the benefit features concerned.

Should
the data shown on your GP DEVA VIP membership card ever
differ from the information recorded in our computer system,
the latter will be deemed to be the correct data, and the
data shown on the membership card will be adjusted accordingly.
- The benefits associated with card membership are to be
enjoyed by the cardholder, and the cardholder alone. We
will verify your membership status when you claim your benefits.
- 10. GP DEVA reserves the right to approve or terminate
your membership for any reason, at our sole discretion.
- If your card is lost or stolen, please contact GP DEVA
Customer Service. Five bonus points will be deducted for
a new membership card.
- Please inform GP DEVA Customer Service by telephone,
fax, or written notice, of any change of address. We cannot
assume any liability for correspondence or e-mail that is
lost, delayed, or misdirected.
